All ten of these use officially accredited guides, which is the qualification Spain requires before anyone may lecture inside a monument. That is the practical difference between a private guide and a private driver: the commentary continues through the Cathedral door rather than stopping at it.
Two of these are quoted for the whole party up to fifteen people. On a price-sorted grid they look expensive; divided by four they are cheaper than the small-group walk. If there are three or more of you, do that division before you compare anything.
Eight of the ten collect you somewhere you name rather than at a fixed kerb. That includes the arrivals platform at the station, which removes the twenty-minute climb into the old town, and your own hotel in Madrid, which removes the journey entirely.
Tell the guide you want the Sephardic history, or El Greco, or the Roman layer under the streets, and the morning is rebuilt around it. Tell them you need to sit down every twenty minutes and it will be. A fixed group route cannot do either.
Most private fares leave the monument admissions out, which sounds like a cost and is actually control: you pay €4 or €12 for the buildings that interest you and walk past the ones that do not, with somebody beside you who can tell you which is which.

You don't get the admissions on most of them, and that is deliberate rather than mean: the wristband and the Cathedral ticket are yours to buy as you go. You also don't get transport to Toledo unless you book one of the two that start in Madrid, or pay the transfer option on one of the others.
Gratuities are excluded in writing on several listings and are normal practice on a private walk. Taxis between distant points are excluded except where the fare names them.
Price band. $116 to $260 a head for a guide inside the city, and $355 to $366 a head when a private car from Madrid is part of it. Two of the in-city fares are per group up to fifteen.
Length. Two hours at the shortest, three as standard, and six or eight on the extended options. Three hours covers the essential circuit; six lets you go inside four or five buildings without hurrying.
Booking window. A few days is usually enough outside Easter and Corpus Christi. Confirmation is immediate unless you book within a day of travel. The train is the thing to book weeks ahead, not the guide.
What to say when you book. Where you want to meet, what interests you, and how far you can comfortably walk. All three change the day, and all three are easier to raise before it than during it.
Solo travellers and couples on a budget. Split two ways, a per-group fare is well above the shared alternatives and you would be paying for exclusivity you may not want. A small-group walking tour caps at ten, costs a fraction, and has the monument tickets in it.

Anyone who has been before and knows the city. Buy the €14 wristband, walk it yourself and spend the money on lunch. Visiting Toledo without a tour makes that case properly.
